Connecting for Better Health Announces Community Trailblazer Award Recipients

Nonprofit dedicated to advancing data exchange highlights individuals driving better care and outcomes for Californians

LARKSPUR, Calif. – March 3, 2025 – Connecting for Better Health (C4BH), the nonprofit coalition dedicated to advancing data exchange in California, today announced the four recipients of its inaugural Community Trailblazer Award, which celebrate individuals or organizations that lead the way in fostering innovation and meaningful change to positively empower communities through data sharing and collaboration. The 2025 award recipients are Hakeem O. Adeniyi, Jr., MD, chief clinical officer of the Sacramento Native American Health Center (SNAHC); Michelle Kuppich, executive director of the California Food is Medicine Coalition; William York, president & CEO of the San Diego Health Connect, 211 San Diego/Community Information Exchange; and Anwar Zoueihid, the vice president and chief strategy officer at Partners in Care Foundation

Awardees will be formally recognized at the Connecting for Better Health Annual Dinner which will be held March 19, 2025 at the Tsakapoulos Library Galleria in Sacramento, California.  Dinner is open to all organizations participating in health and social data sharing, and tickets can be reserved here. C4BH is the state’s leading foundation for data exchange collaboration and facilitation. The nonprofit educates its members and supports meaningful discussion on policy developments, technology advancements, and state and federal policy. C4BH is also responsible for the Data Exchange Framework (DxF) Community Sandbox. This first-of-its-kind virtual testing environment allows health and social service organizations participating in the DxF to seamlessly plan, design and adjust workflows to fully realize the value and promise of the DxF.
