- Individual Supporter Responsibilities
- Program Contribution: Individual Supporter has elected to make a contribution of $500.00 to Connecting For Better Health (C4BH) to support it in achieving our charter objectives.
- Name/Logo: Sponsor authorizes and grants C4BH a limited right to reproduce the Individual Supporter’s name and logo for the purpose of referencing Individual Supporter as a contributing sponsor to C4BH on websites operated by C4BH, as well as in presentations, coalition meetings, and other marketing collateral published and distributed by C4BH.
- C4BH Responsibilities
- Use of Contribution Funds: All contribution funds received by Individual Supporter under the terms shall be devoted within the scope of C4BH’s charitable purposes. C4BH will not use any portion of the funds in a manner prohibited by Section 501(c)(3) of the Internal Revenue Code and applicable California law.
- Tax-Exempt Status: C4BH has submitted an application to obtain a determination letter from the IRS of its Section 501(c)(3) tax-exempt status. Individual Supporter understands and acknowledges that the contribution to C4BH will not be tax-deductible unless and until C4BH receives such determination from the IRS.
- Individual Supporter Recognition and Benefits: The contribution amount made by Individual Supporter entitles the following benefits from C4BH, including:
- Access to Office Hours
- Inclusion of Individual Support’s logo on C4BH’s website
- Invite to End-of-Year Dinner
- Miscellaneous
- Term: The term of this Agreement shall be for the period of one (1) year, commencing as of the date payment received.
- Entire Agreement: This Agreement sets forth the entire understanding between the Parties hereto and supersedes all other prior agreements between the Parties. Each Party to this Agreement acknowledges that no representations, inducements, promises or agreements, orally or otherwise, have been made by any party, or anyone acting on behalf of any party, that are not embodied in this Agreement, and that no other agreement, statement or promise not contained in this Agreement shall be valid or binding as between C4BH and Individual Supporter.
- Counterparts: This Agreement may be executed in one or more counterparts, each of which shall be deemed an original, but which, taken together, shall constitute one Agreement.
- Governing Law: This Agreement shall be governed by and construed in accordance with the laws of the state of California.
